They came from hives all over the place in comic 6 Tahu falls into the Tahnok swarm hive remember.

Moving to Storyline...
They will have all come from the hives, but I'm not sure they're all over the place. They could have been around the Lehvak.
I think there were a bunch of Lehvak and next to them were the other breeds of bohrok. So they could start from the center of Mata Nui and then spread out to attack it. 
I think there were six doors to each of the swarm hives and Takua went into the Lehvak hive.
Just because they aren't featured in the MNOLG, the other Bohrok swarms do indeed awaken, as BIONICLE.com has shown.
