I'm out of med from the doctor.

Way to indirectly explain things. Actually you should go a whole week without sleep. That way you'll be extra tired. So tired that you actually fall asleep when you are supposed to be awake. Also possibly death.

I'm a real insomniac and I need a lot of medicine to get to sleep but I'm out of the medicine I need.

I was awake for almost four days once and I'm never going to try going anywhere close to that.

Sleep deprivation experiments yield wonderful and often shockingly frightening results... if one is paying attention during it.
