I've decided to take requests for avvies and banners!

If you want one, you can do one of two things: if you want most of the creativity to be up to me, you can fill a short form. If you want more control, fill out the second, longer form.

First form:

Simplistic would be just the image with a black border. Effects would include that with other special effects.

Second form:

Remember, be specific about what you want, anything you don't tell me will be improvised!

Credit is not necessary, but it is appreciated.

NOTE: Anything you request will be free for use by anybody else.
